Hospice-friendly trip USA > Japan. What would this theoretically cost?

Dad's entering hospice and I'm considering treating him/us to one last trip. Japan has a place in his heart (traveled around there in his youth) so I'm considering what that would theoretically look like if the opportunity arises. Given current mobility and health limitations I imagine it'd :

- business or first class open-jaw flights (from SFO)

- hotels in central locations, higher-end dining

- paid personal driver and vehicle for the entire trip

- local rental of medical supplies, i.e. oxygen, wheelchair, walker

To make such a trip meaningful (visit enough cities and have enough experiences, i.e. not leave after a week) what itinerary would you suggest, and what price tag am I looking at, i.e. 20-30k USD all-in? I'd also imagine travel insurance is out of the question, but that's another concern.
